I WILL NOT BE ABLE TO ATTEND THE REQUIRED CHECK-IN OR BACK NUMBER CEREMONY. I UNDERSTAND THAT IT IS MY RESPONSIBLITY TO COMMUNICATE WITH THE SECRETARY TO RECEIVE APPROVAL FOR MY ABSENCE AND TO MAKE ACCEPTABLE ARRANGEMENTS TO RECEIVE MY BACK NUMBER, JACKET, ETC. BEFORE I WILL BE ALLOWED TO COMPETE.
